Cuantificación digital de la integración en injertos de piel de espesor parcial ocluidos con sistema de presión negativa

dc.description.abstract | Introducción y objetivo: El éxito en la integración de los injertos de piel de espesor parcial depende de muchos factores. Uno de los más importantes es el método de curación que se le realiza al injerto de piel; por tal razón, el objetivo de nuestro estudio es demostrar el porcentaje de integración que tienen los injertos de piel de espesor parcial a los que se realizó curación oclusiva con el uso del sistema de presión negativa, a través de un programa digital informático. Materiales y métodos: El desarrollo de la investigación se realizó basado en un estudio descriptivo retrospectivo. Se tomó como muestra a todos los pacientes con defectos de cobertura, que presentaban un lecho receptor con tejido de granulación óptimo para realizar el injerto de piel de espesor parcial, en el servicio de cirugía plástica de la Clínica Reina Catalina sede Barranquilla y Baranoa en Colombia, desde el 1 de enero de 2019 hasta 31 de diciembre del mismo año. Para lo anterior, se utilizó el sistema de presión negativa WoundPro® ( © 2020 Pensar Medical, LLC) y posteriormente se cuantificó la integración de los injertos de forma digital, a través de un programa de procesamiento de imágenes (ImageJ), complementado con un desarrollo informático del área de Sistemas de la clínica para obtener un resultado matemático que permitiera de manera más objetiva, evaluar la integración del injerto. El sistema de presión negativa se aplicó durante 6 días, para luego retirarlo y proceder a la toma de imagen fotográfica, la cual se utilizó como insumo para el cálculo de la integración, posterior a la primera curación el paciente es egresado, se realiza un seguimiento de forma ambulatoria, con una media de curación del área injertada de 17 días. Resultados: Se revisaron 70 pacientes, de los cuales el género masculino fue el más afectado con 42 casos, la edad promedio de 41.8 años, la etiología más común que motivaron los injertos fue quemadura en un 34%, el área anatómica de mayor incidencia fue la pierna con un 27.7% y el porcentaje promedio de integración calculado con el desarrollo informático fue de 98,4%. | spa |
dc.description.abstract | Introduction and objective: Success in the integration of partial thickness skin grafts depends on many factors. One of the most important factor is the healing method performed on the skin graft. For this reason the objective of our study is to demonstrate the percentage of integration of the partial thickness skin injections that underwent occlusive healing with the use of the negative pressure system, through a digital computer program. Material and method The development of the research was carried out based on a retrospective descriptive study. All patients with coverage defects, who had a wound with optimal granulation tissue to perform the partial thickness skin graft, were taken as a sample in the plastic surgery service of the Reina Catalina Clinic in Barranquilla and Baranoa in Colombia, from January 1, 2019 to December 31 of the same year. For this, the WoundPro® negative pressure system (© 2020 Pensar Medical, LLC) was used and then, the integration of the grafts was digitally quantified, through an image processing program (ImageJ), and was complemented with a computerized developer of the Systems area of the Clinic to obtain a mathematical result that would allow, more objectively, to evaluate the integration of the graft. The negative pressure system was applied for 6 days, then it was removed and the photographic record is made, that image was used for the calculation of integration, after the first cure the patient is discharged, a follow-up is performed on an outpatient basis, with a healing average of the grafted area of 17 days. Results 70 patients were part of the study, the male gender was the most affected with 42 cases, the average age was 41.8 years, the most common etiology was burn in 34%, the anatomical area with the highest incidence was the leg with 27.7% and the percentage integration average calculated with the digital computer program was 98.4%. | eng |
