Resumen
Introducción: a pesar de los continuos avances en el manejo de la vía aérea difícil
para los médicos especialistas en anestesiología y otras áreas de la salud que se
ven enfrentados a este tipo de pacientes, la habilidad de lograr una ventilación
exitosa en dicho escenario sigue siendo todo un reto. Si bien se trata de una
condición de baja frecuencia en la población general, enfrentarse a una vía aérea
difícil es una situación compleja que puede poner en riesgo la vida del paciente. Con
el objetivo de garantizar un mejor rendimiento en el abordaje de la vía aérea se han
diseñado múltiples dispositivos como videolaringoscopios que facilitan la
visualización de las estructuras de la laringe y aseguran la ventilación adecuada del
paciente tanto en situaciones de urgencia como en cirugías electivas. Sin embargo,
es frecuente que este tipo de equipos médicos sean comercializados a costos
elevados que no permiten la adquisición por parte de personal en entrenamiento o
en áreas de bajos recursos.
Objetivos: los residentes de anestesiología de la Universidad Simón Bolívar
crearon un dispositivo adaptable a sistemas de celulares inteligentes que facilitan la
intubación orotraqueal denominado dispositivo CONVERT®. La utilidad del
dispositivo ha sido evaluada en escenarios de simulación, pero las muestras han
sido limitadas por lo que el presente estudio propone la comprobación del
rendimiento del dispositivo en escenarios de simulación por parte de personal
entrenado y sin entrenamiento.
Metodología: diseño cuasiexperimental que evalúa el rendimiento del dispositivo
“CONVERT®” de videolaringoscopia frente a laringoscopia convencional, por
personal médico en entrenamiento en escenarios de simulación.
Resultados y conclusiones: el uso del dispositivo CONVERT® en escenarios de
simulación por parte de estudiantes de medicina de octavo semestre y residentes
de anestesiología de la Universidad Simón Bolívar demostró una frecuencia de
intubación orotraqueal exitosa en el 87.3% de los casos, adecuado acoplamiento
del equipo y satisfacción “excelente” del uso del dispositivo en la mayoría de los
participantes. Se logró demostrar en laboratorio de simulación, que la mayoría de
los participantes lograron la intubación orotraqueal con el uso del CONVERT®
simulación de vía aérea fácil y 84.2% de los participantes logró asegurar la vía aérea
difícil de su paciente simulado con la ayuda del videolaringoscopio
Introduction: Despite the continuous advances in the management of the difficult airway for physicians specializing in anesthesiology and other health areas who are faced with this type of patient, the ability to achieve successful ventilation in this scenario remains a challenge. While this is a rare condition in the general population, dealing with a difficult airway is a complex situation that can be life-threatening. In order to guarantee better performance in the approach to the airway, multiple devices have been designed, such as video laryngoscopes, which facilitate the visualization of the structures of the larynx and ensure adequate ventilation of the patient both in emergency situations and in elective surgeries. However, it is common for this type of medical equipment to be marketed at high costs that do not allow acquisition by personnel in training or in low-resource areas. Objectives: Anesthesiology residents at Universidad Simón Bolívar created a device adaptable to smartphone systems that facilitate orotracheal intubation called the CONVERT® device. The usefulness of the device has been evaluated in simulation scenarios, but the samples have been limited, so the present study proposes the verification of the performance of the device in simulation scenarios by trained and untrained personnel. Methodology: quasi-experimental design that evaluates the performance of the "CONVERT"® videolaryngoscopy device compared to conventional laryngoscopy, by medical personnel in training in simulation scenarios. Results and conclusions: The use of the CONVERT® device in simulation scenarios by eighth-semester medical students and anesthesiology residents at the Simón Bolívar University demonstrated a frequency of successful orotracheal intubation in 87.3% of the cases, adequate coupling of the equipment, and "excellent" satisfaction with the use of the device in most participants. It was demonstrated in the simulation laboratory that most of the participants achieved orotracheal intubation with the use of the CONVERT easy airway simulation and 84.2% of the participants managed to secure the difficult airway of their simulated patient with the help of the® video laryngoscope.
