Background Adiponectin and leptin are pivotal in the regulation of metabolism. Pediatric lupus nephritis (pLN),
a manifestation of childhood systemic lupus erythematosus (SLE) affecting the kidneys, is associated with impaired
adipokine levels, suggesting a role in pLN pathogenesis. The aim of this study was to explore the potential relationship
between specific single-nucleotide polymorphisms (SNPs)—methylenetetrahydrofolate reductase (MTHFR)
rs1801131 and fibrinogen gamma chain (FGG) rs2066865—and the serum levels of leptin and adiponectin in patients
with pLN.
Methods Ninety-eight pLN patients and one hundred controls were enrolled in the study. Serum leptin and adiponectin
levels were measured using ELISA. DNA extraction and real-time PCR genotyping were performed for MTHFR
rs1801131 and FGG rs2066865 SNPs.
Results Compared to healthy controls, pLN patients exhibited significantly greater serum leptin (11.3 vs. 18.2 ng/
mL, p < 0.001) and adiponectin (18.2 vs. 2.7 ug/mL, p < 0.001). Adiponectin levels were positively correlated with proteinuria
(p < 0.05), while leptin levels positively correlated with proteinuria, SLE disease activity index-2000 (SLEDAI-2K),
and cyclophosphamide usage (all p < 0.05). There was no significant association between MTHFR rs1801131 or FGG
rs2066865 SNPs and pLN in either codominant or allelic models (all p > 0.05). However, the AG genotype of FGG gene
rs2066865 SNP was significantly associated with high leptin levels (> 15 ng/mL) (p = 0.01).
Conclusion Serum adiponectin and leptin levels are associated with pathological manifestations of pLN. High leptin
levels are associated with the AG genotype of FGG rs2066865 SNP in pLN patients, suggesting direct involvement
in disease progression and potential utility as a disease biomarker.

Los pacientes con Enfermedad Renal Crónica (ERC) suelen presentar gran variedad de trastomos en el Sistema Nervioso Central y Periférico que han sido ampliamente descritos en la literatura médica. Sin embargo, su abordaje solo es realizado cuando los signos y síntomas son evidentes, esto ocasiona que la intervención sea solamente paliativa.
Objetivo: Establecer grado de deterioro cognitivo primario en pacientes con enfermedad renal crónica en terapia dialítica.
Materiales y métodos: Se realizó un estudio multicéntrico (3 IPS), transversal. A los pacientes, previo consentimiento informado, se les aplicó el cuestionario para Evaluación Cognitiva De Montreal (MoCA), así mismo se registraron los datos de últimos paraclínicos
(Hemoglobina, Sodio, BUN, Creatinina Sérica), KtV, Fecha de la primera diálisis, enfermedad de base y comorbilidades.; Patients with Chronic Kidney Disease (CKD) often present a variety of disorders in the Central and Peripheral Nervous System that have been widely described in the medical literature. Objetivo: La enfermedad renal poliquística (ERP)
es una enfermedad autosómica dominante causada
por la mutación principalmente de los genes PKD1
y PKD2, y la formación de quistes progresivos en el
tiempo. Este estudio caracteriza los aspectos clínicos
y epidemiológicos de pacientes con ERP atendidos en
un centro de referencia. Métodos: Estudio de corte
trasversal retrospectivo. Se obtuvo 54 registros de
pacientes con ERP. Se analizaron datos demográficos,
clínicos, bioquímicos, imagenológicos y terapéuticos
mediante el uso del software (SPSS 23). Resultados:
La edad media fue 56,9±12,8 años, el promedio de
edad al diagnóstico 48,2 años; sexo predominante
masculino (61 %). El síntoma clínico más frecuente el
dolor abdominal (31 %); la manifestación extrarrenal
más encontrada fue la poliquistosis hepática
(24 %); el estadio renal II fue el más común (24 %),
el antecedente predominante fue la HTA (26 %).
Presentaron antecedente de ERP (16 %); el tratamiento
habitual fue ARA II (34 %). 24 % de los pacientes
recibieron terapia de remplazo renal. Conclusiones:
La ERP afecta principalmente a personas de 56 años,
predominio masculino. Entre los hallazgos clínicos
se encuentra el dolor abdominal; las manifestaciones
extrarrenales más frecuentes: poliquistosis hepática,
hernias y la enfermedad diverticular; la HTA constituye
el antecedente más frecuente. La hemodiálisis fue la
modalidad sustitutiva más usada.; Objective: Polycystic kidney disease (PKD) is
characterized by mutations in PKD1 and PKD2 and
progressive cyst formation over time. This study
characterized the clinical and epidemiological aspects of patients with PKD treated at a reference
center. Methods: Retrospective cross-sectional
study. We obtained 54 records from patients with
PKD. Demographic, clinical, biochemical, imaging
and therapeutic data were analyzed with the software
(SPSS 23). Results: The mean age was 56.9±12.8
years, and the mean age at diagnosis was 48.2 years,
predominantly male (61 %). The most frequent
clinical symptom was abdominal pain (31 %);
polycystic liver disease (24 %) was the most common
extrarenal manifestation; renal stage II was the most
common (24 %); and the predominant antecedent
was hypertension (26 %). 16 % of the patients had
a history of PKD (16 %), the usual treatment was
ARB II (34 %). 24 % of the patients received renal
replacement therapy. Conclusions: PKD mainly
affects 56-year-olds, predominantly male. Clinical
findings include abdominal pain. The most frequent
extrarenal manifestations are polycystic liver disease,
hernias, and diverticular disease; HTA is the most
frequent antecedent. Hemodialysis is the most used
replacement modality.

Introducción: La enfermedad renal crónica (ERC) se define como un conjunto de enfermedades heterogéneas que afectan la estructura y función renal. Esta revisión se realiza con el fin de identificar y recopilar información de los marcadores inflamatorios que se utilizan para el seguimiento, mortalidad, pronóstico de pacientes con ERC, incluso los que se usan para predecir dicha patología renal. Objetivos: Recopilar datos sobre los marcadores inflamatorios inmunológicos asociados con la ERC mediante la revisión de artículos de investigación para determinar si existen predictores inmunológicos específicos de dicha patología crónica. Métodos: Se realizó una revisión estructurada en la base de datos
Medline/ PubMed, usando los términos: Renal Insuficiency, Chronic OR Acute-Phase Proteins OR Monitoring, Immunologic. Los artículos publicados entre 31/12/ 2009 y 31/12/2018, en ingles con acceso al texto completo. Resultados: se encontraron en total con los términos MeSH, 2.850 artículos, de los cuales se excluyeron 1930 por no presentar una relación de utilidad clara entre de los marcadores inmunológicos y la ERC, y otros 895 por su principal enfoque en otras patologías a nivel renal como la insuficiencia renal aguda. Por lo tanto, para este trabajo se seleccionaron y revisaron 25 referencias con estudios entre el año 2003 al 2019.; Introduction: Chronic Kidney disease (CKD) is defined as a set of heterogeneous diseases that affect renal sfructure and frnction. This review is conducted to identify and collect information from inflammatory markers that are used for the follow-up, mortality, prognosis of patients with CKD, including those used to predict such renal pathology
Objective: To collect data on immunological inflammatory markers associated with the CKD by revising research articles to determine whether there are specific immunologic predictors of such chronic pathology.
Materials and methods: A structured review was conducted in the Medline/PubMed database, using the terms: Renal Insuficiency, Chronic OR Acute-Phase proteins OR Monitoring, Immunologic. The articles published between 31/12/2003 and 31/12/2019, in English with access to the full text.
Results: were found in total with the MeSH terms, 2,850 articles, of which 1930 were excluded because they did not present a clear useful relationship between the immunological markers and the CKD, and another 895 for their main focus on Other pathologies at the renal level such as acute renal failure. Therefore, for this work, 25 references with studies between 2003 and 2019 were selected and reviewed.
